Documentation Requirements for Volunteers

Tulalip Little League requires supporting documentation in addition to the online registration form due at the time of Spring registration. New volunteers in our Fall program will be required to provide proof of address at the time of registration.

Documentation will be collected during our Registration Turn-in Days which will be happening throughout the registration period.

All Volunteers

All volunteer applicants are subject to a background check which requires:

  1. Driver's License - a copy will be kept on file. If you have submitted your Drivers License as a part of your player's proof of address, it does not need to be re-submitted.
  2. Social Security Number: Download here a copy of the Little League Volunteer form. Complete your name, social security number, and signature. (All other information has been collected during the online registration.)
  3. Optional: Certification information, such as CPR training

Applicants for Manager, Coach or Practice Helper

Washington state requires all adult volunteers to complete training in Concussions and Sudden Cardiac Arrest. Tulalip Little League requires all manager, coach, and practice helper applicants to submit a certificate of completion for each.

  1. Heads Up Concussion Training - annually
  2. Sudden Cardiac Arrest Training - every three years. Certificate must be valid for the entire season which is June 30th for Spring Season; August 30th for All Star/Tournaments: and October 30th for Fall Ball.
